Residential Services
TLC Connection offers host homes, group homes, personal care alternatives, and respite services. All services are provided by caring families who welcome individuals with intellectual and developmental disabilities into their home and support them with care, dignity, & respect in a safe and supportive living environment.
All providers assist with daily living activities and independent life skills such as shopping, money management, and cooking.
HOST HOME SERVICES
Host home services allow individuals to live in a family setting with one to three individuals in services. Each host home has extensive background with the I/DD population and has extensive training. Our person-centered approach provides individuals with the opportunity to set goals that they feel are meaningful and our providers assist them to meet and exceed those goals.
*All host home services offer 24 hour support. Host home families and individuals are matched according to likes and similar ideals. All host homes receive monthly review to ensure the homes are safe, and to ensure the needs of our individuals are being met.
FAMILY CAREGIVER SERVICES
TLC Connection offers support to individuals who would prefer to live and be supported by family members or friends. Our agency will hire on your caregiver and help them gain the necessary training in order to become your host home provider. Our agency compensates family caregiver providers at 70% of the daily host home rate. We provide additional support to all family caregiver host home providers to ensure their success.
Each family caregiver home is reviewed on a monthly basis to ensure that all proper protocols and supports are being received. We would love to help your family achieve their goals of supported community integration!
RESPITE SERVICES
Respite Services are also available through TLC Connection. Whether you are a host home provider, family caregiver, or a caregiver of a family member of the HCBS waivers, we can assist you in finding the perfect fit. All providers have the necessary training, background, & experience in order to provide individuals with the best care possible. Respite is provided by individuals or families who can give short-term relief to families or caretakers who need a break for caring for their loved ones with special needs on a temporary basis.
PERSONAL CARE ALTERNATIVE SERVICES
TLC Connection offers personal care alternative services to provide residential opportunities to individuals on a more independent level. Individuals can choose to live in their own residential settings (individual owned or rented properties) or can live in one of the agency owned apartment or condos. Staff members provide support to individuals and ensure that all daily living activities are completed. Each individual will work with staff to set goals that will increase and hone daily living skills to ensure independent living settings are appropriate. Staff are on call 24/7 to ensure health and safety of each individual.
GROUP HOME SERVICES
TLC Connection offers group home settings that are fully staffed. These residential settings offer extensive supports with awake staff 24/7. Each group home staff member has extensive background with the I/DD population and has extensive training. Our person-centered approach provides individuals with the opportunity to set goals that they feel are meaningful and our providers assist them to meet and exceed those goals.
