TLC Connection offers several residential services to meet your unique needs. We match individuals with host homes based on their likes, extracurricular activities, and supervision levels. All potential individuals are provided host home profiles to get to know our providers and are encouraged to schedule a meet and greet to see the home in person. TLC Connection offers Supported Community Connection Services to individuals enrolled on the DD and SLS waiver. The program is completely community based and is tailored to help individuals learn life skills while encouraging community integration. Services are offered in a one on one and group setting. Transportation is provided to all individuals. Here are a couple of places our program visit:
